payment options

Full payment or co-payment is due at the time of service. In addition, we reserve the right to charge for appointments cancelled or broken without 24 hours advance notice.

We accept many forms of payment, including: Visa, MasterCard, Discover, American Express, and Debit Cards. We also offer several financing options, including the Orthodontists Fee Plan and Wells Fargo. For orthodontics, we also provide Patient Budget Plans.

We are happy to file insurance claims and we are preferred providers with Aetna, Blue Cross Blue Shield, Delta Dental, Educators, Jefferson Pilot, MetLife, PEHP, United Concordia, and the Utah Local Government Trust.
